Not directly answering any of your questions, but one of the things
which made markdown popular is that in its raw form, it is easily
readable. Headers are defined by how they are underlined, for example,
and lists just look like lists in raw text (numbered or not).
This also makes them easier to edit.

On the other hand, there have been many "improvements" which enable
"more complete" HTML conversion (markdown didn't originally have
anything which converted to tables, for instance) and these improvements
typically make the format less readable.
