OK folks, I'm about to start this change. I expect it to take a couple of
hours.

It is going to be a little disruptive and will cause some breaks before I'm
done, as the changes span 5 repos. The repos themselves will be renamed as
well:

libbitcoin_blockchain -> libbitcoin-blockchain
libbitcoin_client -> libbitcoin-client
libbitcoin_explorer -> libbitcoin-explorer
libbitcoin_protocol -> libbitcoin-protocol
libbitcoin_server -> libbitcoin-server

In the interest of consistency I will also change _test to -test.

There will be no impact to libbitcoin, obelisk or sx.

Given that these repos are all pre-release there is no "public" impact.
