I've only ever seen it used to designate the originating author
considers that it is somewhat "Off Topic" to the general population of a
given mailing list (or newsgroup).

Usually I would expect that someone posting off topic to mainly be
looking for the right step to take to take their question somewhere that
it is on-topic.
