I happens that the major number jumps above 254, as in the case of LVM
partitioning. For example:

8:48     /dev/sdd

8:49     /dev/sdd1

8:50     /dev/sdd2

...

8:63     /dev/sdd15

259:0   /dev/sdd16   <-----  HERE

259:1   /dev/sdd17

259:2   /dev/sdd18

...
